DOCTORS SAMPLE

Below are sample letters to enhance your emotional support dog kit. If you are planning a trip which includes flying or moving into a residence where animals are not normally allowed simply provide your treating physician with a sample letter to sign and present the letter along with your ID card.

 

AIRLINE SAMPLE LETTER: Prescribes an Emotional Support Animal specifically for airline flights (the letter should be on the physician’s or mental health provider’s letterhead)

To Whom It May Concern:
[Full Name of Tenant] is my patient, and has been under my care since [date]. I am intimately familiar with his/her history and with the functional limitations imposed by his/her emotional/mental related illness.

Due to this emotional/mental disability, [first name] has certain limitations related to [social interaction/coping with stress/anxiety, etc.]. In order to help alleviate these difficulties, and to enhance his/her ability to function independently, I have prescribed [first name] to obtain a pet or emotional support animal. The presence of this animal is necessary for the emotional/mental health of [patient name] because its presence will mitigate the symptoms he/she is currently experiencing.

Please allow (patient’s full name) to be accompanied by his/her emotional support animal in the cabin of the aircraft, in accordance with the Air Carrier Access Act (49 U.S.C. 41705 and 14 C.F.R. 382).

I am licensed by the state of (state) to practice (medicine/psychiatry/therapy–choose applicable). My license number is (license number).

GENERAL SAMPLE LETTER: Prescribes an Emotional Support Animal (the letter should be on the physician’s or mental health provider’s letterhead)

To Whom It May Concern:
[Full Name of Patient] is my patient and has been under my care since [date]. I am intimately familiar with his/her history and with the functional limitations imposed by his/her emotional/mental health-related issue.

Due to this emotional disability, [patient first name] has certain limitations coping with what would otherwise be considered normal, but significant day to day situations. To help alleviate these challenges and to enhance his/her day to day functionality, I have prescribed [patient first name] to obtain an emotional support animal. The presence of this animal is necessary for the emotional/mental health of [patient name] because its presence will mitigate the symptoms he/she is currently experiencing.

HOUSING LETTER SAMPLE: Specifically prescribes an Emotional Support Animal for housing situations (the letter should be on the physician’s or mental health provider’s letterhead)

Dear [Name of Housing Authority/Landlord]:
[Full Name of Tenant] is my patient, and has been under my care since [date]. I am intimately familiar with his/her history and with the functional limitations imposed by his/her emotional/mental related illness.

He/She meets the definition of disability under the Americans with Disabilities Act, the Fair Housing Act, and the Rehabilitation Act of 1973.

Due to this emotional/mental disability, [first name] has certain limitations related to [social interaction/coping with stress/anxiety, etc.]. In order to help alleviate these difficulties, and to enhance his/her ability to live independently and to fully use and enjoy the dwelling unit you own and/or administer, I have prescribed [first name] to obtain a pet or emotional support animal. The presence of this animal is necessary for the emotional/mental health of [patient name] because its presence will mitigate the symptoms he/she is currently experiencing.

Note: Service Dogs do not require a note from any doctor however for those that are in need of a prescription letter for ESA we will supply you with a few reputable providers when you register.

People with service animals must be allowed access to all public accommodations.
This right takes precedence over all state and local laws which might otherwise prohibit animals in those places such as Stores, Malls, Restaurants, Hotels/Resorts, Airlines,Cruises, Taxi cabs, Buses just to name a few.

Many landlords and business owners request to see registration paperwork or other identification and even though such paperwork is not required under law, it is often easier to provide the requester with a registration document or ID rather than argue the point or attempt to educate the person.

Emotional support dogs are protected under federal law Under the Fair Housing Amendments Act (FHAA) and the Air Carrier Access Act (ACAA), protects an individual who is entitled to an emotional support dog to assist them with their life.

The Fair Housing Amendments Act of 1988 (42 USC 3601, et seq.) prevents landlords and home owner associations from restricting your Emotional Support Animal from living with you.(even when there is a no pet policy in place) They are also not allowed to access special pet fees on you because of your pet.

The ACAA protects individuals by allowing the emotional support dog to fly with them in the cabin of an airplane (without having to pay any additional fees).

Any size or breed of dog can be an emotional support or service dog, emotional support dogs do not have to be professionally-trained to perform any task.
